The Evil Within is an action-survival horror game inspired by the Resident Evil series, featuring a grimy look with exaggerated blood. The player controls detective Sebastian Castellanos, who is called to the scene of several murders at Beacon Mental Hospital along with his partners Joseph Oda and Julie Kidman. When they arrive, they find all the police cars abandoned, and almost everyone has been brutally killed. While reviewing security footage, he spots a hooded man named Ruvik. In the next moment, the man is behind him, and Castellanos passes out. He wakes up in a world he doesn't recognise, held captive in the basement of a huge man wielding a chainsaw. After escaping, he investigates what happened across sixteen chapters in various locations.
As a survival horror game, Castellanos is often outnumbered, enemies are strong and supplies are scarce. Many confrontations can and should be avoided by sneaking. Enemies have an awareness level and can be lured away by throwing a bottle. Eye icons indicate when an enemy is nearby or has been alerted. A lantern is used to light up dark areas. There are environment-based puzzle elements involving items and notes, and small stone statues can be destroyed to find keys for opening lockers containing items. Castellanos can engage in melee combat and uses standard weapons such as a revolver and a shotgun, as well as grenades. Once the Agony Crossbow is discovered, numerous ways to take out enemies open up, such as freezing or poisoning them, or crafting harpoons or shock traps to stun them. Health can be restored using a limited number of syringes, and matches burn defeated enemies, killing them permanently.
The camera hovers close behind Castellanos. Many level sections offer multiple routes, with the option to sneak silently or hide. While exploring, jars of green gel can be collected, which serve as upgrade points. Looking into a mirror transports Castellanos, in his mind, to an asylum. There, the game can be saved, and there's a chair he can sit in to upgrade weapons and abilities across up to five levels. Points can be spent to upgrade weapons with a damage multiplier, a faster fire rate, a shorter reload time, a larger magazine capacity, improved accuracy or a greater chance of critical hits. They can also create more room for ammo or items, or be used to improve physical abilities, such as more health, longer sprint time, more melee damage or greater syringe recovery.
There are traps and bombs scattered throughout the environment that can be defused to obtain trap parts, which are used to craft crossbow ammo. Traps can also be strategically placed to limit the number of directions from which enemies can attack. Enemies include animals, standard “Haunted” enemies and a range of grotesque creatures. There are also boss fights. There are three difficulty levels.
